Forms and Information
This page allows you to complete all necessary forms, as well as set an appointment with Daphne Riddle, LPC. There are several forms that need to be completed and brought to your first session. Please print, read and sign the Informed Consent, Notice of Privacy Practices, and the HIPAA Signature Page and additional HIPAA forms. You can then go to "Set Appointment" which will take you to the Therapy Appointment website. You can set up a New Patient page where I will need you to complete your biographical and insurance information, if you are using insurance.
